Medtronic received a report that the echelon was shaped with a shaping needle and heated to set the shape. When the shaping needle was pulled out to prepare for the delivery of echelon, it was found that the outer layer of the microcatheter tip showed signs of melting. It was reported catheter rupture occurred during an aneurysm embolization. The catheter was ruptured in the distal section. There was no friction or difficulty during delivery.Aside from the rupture, there was no other damage to the catheter. The microcatheter did not enter the patient. The reported device and any accessory devices were prepared as indicated in the instructions for use (ifu). The catheter was flushed as indicated in the ifu. There is no patient involved in this event.Ancillary devices include a bridge silver snake da guide catheter, transcend guidewire.

H3.Product analysis: equipment used: video inspection system (m-85519), ruler (m-83361), pin gauge sets (m-84083, m-84081), camera (panasonic lumix dmc-zs5), as found condition: the echelon-10 micro catheter was returned for analysis within a shipping box and within a sealed plastic biohazard pouch. The shaping heat source used during the event was not returned for analysis.Visual inspection/damage location details: no damages were found with the echelon-10 micro catheter hub.No damages or irregularities were found with the micro catheter body. The distal ~2.8cm of the catheter exhibit severe tip shaping damage (melting). No damages or irregularities were found with the distal tip or marker band (other than the melting). No rupture was observed.Testing/analysis: the echelon-10 micro catheter total length was measured to be ~155.0cm and the usable length was measured to be ~1 47.2cm, which is within specification (specification: total (ref) = 155cm, usable: 147cm ± 1.5cm).Conclusion: based on the device analysis and reported information, the customer¿s report of ¿catheter rupture¿ could not be confirmed, however, the report of catheter melting was confirmed. It is likely the damages occurred during the tip shaping process.Possible causes of the damages include but not limited to using a heat source other than steam, holding the catheter tip too close to the heat source (1 inch), due to holding the device against the heat source too long (> 10 seconds).Customer reported catheter was held 6-8cm from the heat source and the heat source was used on the catheter 20-30 seconds. It is likely the reported time held within the heat source contributed towards the reported catheter melting.H6.Coding updated based on analysis findings medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
