Additional Manufacturer Narrative
Product complaint #(b)(4).Investigation summary: according to the information received, ¿approximately 4 years post-operation during a routine workout early in 2024, i experienced sudden sever pain and mobility issues in my hip, accompanied by unusual squeaking and loud noises during movement.Diagnostic tests including x-ray and ct scans revealed a failed right total hip replacement and a dislocated polyethylene liner.A revision surgery was necessary and performed on (b)(6) 2024 by the surgeon. The procedure revealed significant damage to my pelvic bone, primarily attributed to the failure of the hardware.Notably, extensive damage was observed in the anterior aspect of the liner and a posterior wall fracture was also identified upon the removal of the acetabular component.Furthermore, there was considerable metallosis in the joint, and the polyethylene liner was found dislocated from the cup. These issues have caused not only significant physical pain and impairment but also considerable emotional and financial distress. The need for revision surgery has further compounded these difficulties.¿ the product was not returned to j&j medtech orthopaedics; however, photos were provided for review.See attachment "(b)(4)".The photographs attached were reviewed, however they do not depict the reported corail2 std size 12.Therefore, the investigation could not draw any conclusions about the reported event due to the insufficient evidence provided. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was not confirmed as the photographs provided contained insufficient evidence of the reported event.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.H11 additional narrative: corrected: d4 (primary udi number).
