Event or Problem Description
Physician attempted to use a microvascular plug 9q during patient treatment. The mvp plug was soaked in heparinized saline prior to use.The catheter was flushed before inserting the plug.Physician was closing a pulmonary systemic collateral from the innominate vein to the left superior pulmonary vein. The collateral was very tortuous. No resistance was felt when advancing the plug through the introducer sheath or the catheter.Physician advanced the plug and when he wanted to expand the plug, the expansion was suboptimal. The plug was re-sheathed into the catheter for repositioning but at that time the plug was damaged. It was decided to remove the plug.It was re-sheathed and at that moment it was cut, for this reason they had to remove everything and loose the position and do the puncture again. The doctor was able to remove the guide and plug.No strut fracture was observed.A replacement plug was used without any problems to complete the procedure. No medical or surgical intervention was needed to prevent a permanent impairment of a function. No patient symptoms or complications associated with this event and did not lead to or extend patient hospitalization. No patient injury reported.
