The reported event could be confirmed since the device was returned for evaluation and matches the alleged failure. The device inspection revealed the following: during visual inspection both k-wire and cannulated drill were found severely deformed and damaged. Both were severely stuck into each other in the deformed condition. The drill was found broken and the remaining flutes were wrapped around the k-wire.Impressions of the drill are clearly visible on the surface of the k-wire which indicates that the internal drill portion came in exact contact with the k-wire prior to the failure and broke due to application of excess torsional force on it while drilling. This is most probably due to the deformation of the k-wire either prior to drilling because the bone could not be hard and excess force applied while inserting the k-wire or during drilling due to misalignment with the drill itself.Furthermore, a hardness test according to rockwell on the returned drill indicates the material being in accordance with the values in the material specification.A review of the device history for the reported lot did not indicate any abnormalities. No corrective actions are required at this time.A review of the labeling did not indicate any abnormalities. No indications of material, manufacturing or design related problems were found during the investigation.Based on the investigation, the root cause was attributed to a user related issue. The event was caused due to application of excess force on the device when it was misaligned during usage. If more information becomes available, the case will be reassessed.
