Investigation details: using econnectivity, gtsc analyzed the logfiles (column grade report and barcode scan report) from the day of the reported events (30 april 2024) confirming that issue was not due to any analyzer error/fault.Additionally, the order reports provided by the customer shows that patient sample id (b)(6) (patient id not defined) loaded on the analyzer on 30 april 2024 and tested as o d(rh1) antigen negative, was identified with an accession label as patient sample id (b)(6) (patient id (b)(6)), thus, it is concluded that the sample id which was loaded on (b)(6) 2024 was from a different patient than those tested on (b)(6) 2024.It is concluded that the assignable cause of the discrepant abo-blood group and d(rh1) antigen typing obtained for this patient with their ortho vision id-mts analyzer is a preanalytical laboratory error.Gtsc contacted the customer to provide the investigation findings. The customer agreed with the conclusion of the investigation and was able to determine how the error occurred on (b)(6) 2024.No further detail was provided. No further investigation was carried out on these incidences. The assignable cause of the discrepant abo-blood group and d(rh1) antigen typing obtained for one patient with an ortho vision id-mts analyzer is a preanalytical laboratory error. There is no evidence of any systematic failure of the ortho analyzer to perform as intended. No other complaint of this type has been received from the customer site since the time of the reported events.
