udi: (b)(4).With all available information, boston scientific concludes that the reported staphylococcus infection is a known inherent risk as documented in the instructions for use (ifu), however, the cause of the reported impaired healing is unable to be established. The paddle lead and ipg have not been returned by the medical facility as the patient could not remember the name of the physician or hospital where the explant took place, and therefore, the explanted devices will not be returned. As such, physical analysis has not been conducted in our laboratory.A review of the device history record (dhr) for the devices was performed. The review of the dhr identified that there were no process-related non-conformances, scrap, or rework performed during the production that could explain the event. The review ensures each device meets specifications prior to release for use. There is no indication that the device manufacturing process contributed to the reported complaint. The labeling review performed did not reveal any anomalies.Per the instructions for use (ifu), possible surgical procedural risks are: temporary pain at the implant site, infection, cerebrospinal fluid (csf) leakage and, although rare, epidural hemorrhage, seroma, hematoma and paralysis are known risks associated with implantation of a spinal cord stimulation (scs) system. The reported event of a staphylococcus infection at the paddle lead incision site, resulting in explant of the spinal cord stimulation (scs) system, is confirmed; however, a device malfunction or performance issue is not confirmed based on the available information. The device was not returned for analysis as such, physical analysis has not been conducted in our laboratory. The device history record review did not identify any manufacturing deviations that could have contributed to the event.Labeling review identified that infection is a known risk of implanting a pulse generator as part of a system to deliver spinal cord stimulation, however insufficient information is available to determine the underlying cause of the infection.
