Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary - according to the information received, ¿upon inspection by the rep, he managed to free the broken pin and check the cutting block against other pins ¿ this revealed that none of the pins would fit correctly and likely the metal hole had perhaps warped out of shape and would not function correctly.¿ the same pins were compared to the right cutting block on this tray and performed as intended. The product was not returned to depuy synthes; however, photos were provided for review.See attachment (pc-001639585 attune 0 deg left cut block (b)(4)).Review of the photographic evidence revealed that the attune solo pinning system has broken in half. One of the photos shows that a broken fragment was stuck inside the cutting block. The observed condition of the device was consistent with off axis insertion and an exposure to unintended forces, like usage of excessive force in a prying motion during the assembling process. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was confirmed as the observed condition of the attune solo pinning system would contribute to the complained device issue.Based on the investigation findings, the potential cause is traced to unintended use error, and it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot - the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.H11 additional narrative: correction: d4 (primary udi number).
