Block a: the patient's dob or age at time of event, sex, gender, weight, ethnicity, and race are unknown.Block b6/b7: patient information regarding relevant tests/laboratory data or medical history are unknown.Block h3: the tes device was returned for evaluation.Visual inspection found no unusual characteristics of the device.During functional testing, a laboratory 0.014" guidewire was used, but was unable to insert through the device due to an occluded inner lumen. When attempting to deploy the tacks, the outer sheath was stuck and did not move as the pusher shaft was being pushed. The outer sheath was pushed back manually, then it was able to move by using the pusher shaft. The tacks deployed but possible dried contrast was observed on the distal tip.In order to attempt insertion of the guidewire into the lumen of the remaining shaft, the distal tip was cut to remove the occlusion. However, the tip was distorted from being cut and the guidewire was unable to advance without kinking the shaft due to resistance noted.Block h6: the root cause could not be established due to the inability to advance the guidewire during the functional testing.A review of the lhr and manufacturing process could not confirm a cause related to design and/or process failure.Submission of this report does not, in itself, represent a conclusion by the manufacturer and/or authorized representative or the national competent authority that the content of this report is complete or accurate, that the medical device(s) listed failed in any manner and/or that the medical device(s) caused or contributed to an alleged death or deterioration in the state of the health of any person.
