Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary - impaction cup had damaged on the inner rim that is flaking off lps inserter/ extractor - internal rod is bent and had wear and tear scratches on it making it hard to inserter into the holder. The product was returned to depuy synthes for evaluation.Visual analysis of the returned device revealed that the lps inserter extractor had a bent and scratched internal shaft.Additionally, signs of heavy impactions were evident on the surface, along with wear marks across the surface. The bent condition of the shaft can be attributed to a combination of heavy use and exposure to unintended forces, such as impactions during repeated attempts at assembly and disassembly, or while the device was assembled with the mating device. The scratches are consistent with other tools and hard edges coming in contact with the device.Properly handling and attention to the approved use of the device diminishes the risk of failure.Worn found is consistent with the effects of extensive and repeated use, suggesting that the device has been subjected to frequent handling and operation.A functional test was performed, and the handle/shaft could be assembled, however it was difficult to fully assemble. This difficulty is consistent with the internal shaft being bent, which likely impeded smooth assembly. Therefore the reported allegation can be confirmed.A dimensional inspection was not performed since it was not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the lps inserter extractor would have contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot - the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.H11 additional narrative: added: h3.
