Additional Manufacturer Narrative
(b)(4).Date sent: 8/28/2024.D4: batch #448a60.Investigation summary: the product was returned for evaluation.Visual inspection and functional testing were conducted on the returned device.Visual analysis of the returned sample determined that the tlc10 device was received with no apparent damage and with a reload loaded on the device. The reload was received with the swing tab in the unlocked position and left gripping surface broken and not returned making the reload nonfunctional. The reload had the proximal 6 driver up without staples, and the remaining drivers down with staples present. In addition, corrosion and fluids were noted in some parts of metal area and mechanism knob causing the knob stiff. However, the device was cleaned and the device was tested for functionality with a test reload. The device achieved its complete firing sequence without any difficulties. The staple line and cut line were complete and the staples meet the staple form release criteria. The event reported was confirmed and it is related to improper use of the device. As it is possible that the firing knob was not returned to its home position while loading the reload. The condition of the broken gripping surface is consistent with an improper loading technique. When loading the cartridge in the device, make sure the cartridge is inside the channel track and the proper loading technique. As part of ethicon endo surgery¿s quality process, all devices are manufactured, inspected, and released to approved specifications.A manufacturing record evaluation was performed for the finished device batch number 448a60, and no non-conformances related to the reported complaint condition were identified.
