An attempt was made to use one nc euphora balloon catheter to treat a mildly tortuous, mildly calcified lesion in the mid left anterior descending (lad) artery. The device was inspected with no issues noted.Negative prep was performed with no issues noted. The lesion was pre-dilated.The device passed through a previously deployed stent.Resistance was not encountered when advancing the device.Excessive force was not used during delivery. It was noted that 80% saline/20% contrast was used during the procedure. It was reported that balloon deflation difficulties were encountered and the balloon would not deflate at the lesion site.Difficulties were also encountered when removing the device following stent deployment. The balloon was cut and a non-medtronic guide extension catheter was advanced over the balloon to retract the device out of the body. The patient is alive with no further injury.

Additional information: it was not difficult to remove the protective sheath. It was not difficult to remove the packaging stylette. The device passed through a previously deployed medtronic stent. The balloon was being used to post-dilate the stent. No difficulties were noted during inflation of the device. One inflation was performed prior to the deflation difficulties.A nominal inflation was applied for the inflation. The device was not moved or repositioned while inflated.Sufficient time was given to allow the balloon to fully deflate prior to attempting removal. The stent was not damaged or deformed due to the difficulties experienced when removing the balloon. On the removal of the balloon from the stent, resistance was not encountered as the device was retracted over the guidewire. No damage was noted to the guidewire on removal from the patient. It was believed that the stent was sufficiently expanded prior to attempting post-dilation of the stent. There was no issues with the previously deployed medtronic stent.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.

Additional information: two videos were provided from the account. The first video showed the presence of a balloon delivery system in the mid vessel. The second image shows and inflated balloon with a guide extension catheter (gec). The videos do not provide a root cause for the balloon deflation and removal difficulties. The non-deflator appears to be confirmed from the images.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report.
