Model Number 693552
Medical Device Problem Codes Signal Artifact/Noise (1036); Fracture (1260); High impedance (1291); Over-Sensing (1438); Low impedance (2285); Impedance Problem (2950)
Health Effect - Clinical Code No Clinical Signs, Symptoms or Conditions (4582)
Date of Event 11/11/2021
Type of Reportable Event Serious Injury
Event or Problem Description
It was reported that the right ventricular (rv) lead triggered a lead integrity alert (lia) due to sensing integrity counter (sic) and pacing impedance variation. It was also noted that the rv lead triggered alerts for high out of range (oor) superior vena cava (svc) impedance and low oor pacing impedance.Noise was also observed. It was suspected that the lead was fractured. The polarity of the lead was reprogrammed and the lead remains in use. no patient complications have been reported as a result of this event.

Additional Manufacturer Narrative
Continuation of d10: setroxs53 lead implanted: (b)(6) 2010 medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.

Additional Manufacturer Narrative
Product event summary: the lead was not returned for analysis, however, performance data collected from the device was received and analyzed.Analysis of the device memory indicated oversensing due to non-physiologic signals/sensing integrity counter.Analysis of the device memory indicated the criteria for the right ventricular lead integrity alert were met.Analysis of the device memory indicated the impedance of the right ventricular pacing lead was below the expected lower range.Analysis of the device memory indicated the impedance trend of the right ventricular pacing lead was variable.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Medtronic will submit a supplemental report if additional relevant information becomes known.

Event or Problem Description
It was further reported that the rv lead exhibited high pacing and superior vena cava (svc) coil impedance measurements. The lead was extracted and replaced. No patient complications have been reported as a result of this event.
