A visual, dimensional, and functional analysis were performed on the returned device. The reported difficulty advancing the device over the guide wire, break or material too soft/flexible (flimsy) could not be confirmed.Production record and corrective and preventive actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a review of the complaint history identified no other similar complaints from this lot.Based on the reported information and results of the complaint investigation there is no indication the reported difficulty advancing the device over the guide wire, break or material too soft/flexible (flimsy) are related to a potential product quality issue.Possible factors that may contribute to the difficulty positioning the bdc over the guide wire may include, but not limited to, device placement technique, guiding catheter support, inner diameter of guide wire lumen, condition of the guide wire, or damage to the distal shaft of the catheter.Returned device analysis noted multiple kins and bends along the outer member and hypotube which possibly resulted from inadvertent mishandling of the device during preparation; however, this cannot be confirmed. In this case, it is possible that the reported resistance resulted from the noted kinks and bends on the device and subsequently gave the impression of the device feeling flimsy and broken; however, since the reported complaints could not be confirmed during return analysis, a conclusive cause cannot be determined. There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.

It was reported that when the 3.0x30 mm trek balloon dilatation catheter (bdc) was being advanced onto the guide wire and resistance was noted. The guide wire was inside the anatomy while the balloon was advanced. The distal shaft of the trek bdc was noted to be broken but remained in one piece. There were no reported adverse patient effects, and there was no reported clinically significant delay in the procedure. An unspecified balloon was used to complete the procedure.Subsequent to the initially filed report, it was reported the shaft felt flimsy during advancement. No additional information was provided.
