This follow-up report is being submitted to relay additional and/or corrected information. The following sections were updated/corrected: d1, h6 mdr section codes updated/corrected: b, c, d, g the revision reported was likely the result of torque screw disassembly leading to humeral tray disassociation. The cause of the disassembly cannot be confirmed from the information provided, but may be related to incomplete seating during implantation, fracture, or abnormal forces resulting in the screw backing out over time. However, the series of events cannot be confirmed and potential contributions of user or patient-related considerations to the event cannot be confirmed as the devices were not returned for evaluation and product images and initial post-operative radiographs were not provided. If any further information is obtained that would change or alter any information provided, a supplemental report will be filed accordingly.
