Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: it was reported that the torque wrench got stuck on the adapter. It was unknown, if there was a surgical delay. No further information is available. The product was returned to j&j medtech orthopaedics for evaluation.Visual inspection revealed hex edges of the sig fem adpt torque wrench deformed and with a scratch located in the midline.Potential cause can be attributed to unintended use error by improper engagement of the mating components, causing deformations on its edges and subsequently causing a difficulty on the assemblance of device.A dimensional inspection was not performed since it was not applicable to the complaint condition.A functional test was unable to be performed as the mating component was not returned for evaluation. However, based on the damage observed on the torque wrench, it is reasonable to confirm a difficulty on the assemblance of the device.Potential cause can be traced to a component failure, as condition may had happen as a consequence of the observed damage. The overall complaint was confirmed as the observed condition of the sig fem adpt torque wrench would have contributed to the complained device issue.¿ based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
