On september 04, 2024, the mentor analysis lab received the suspect medical device for evaluation. On september 17, 2024, mentor completed an evaluation on the returned device.Mentor conducted visual inspection of the device.Visual analysis of the returned sample revealed that the breast implant was found to have a tear on the posterior view measuring approximately 1.1 cm.Additionally an area of siltex cracking was observed on the edges of the tear. The evaluation determined that the possible cause of the rupture is consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses.Granulomas are noncancerous nodules that are a common tissue reaction to the presence of a variety of foreign materials, such as silicone.Manufacturer¿s reference number: (b)(4).

On november 19, 2024, mentor received the following additional information: the patient was originally diagnosed with the ruptured right breast implant with a suspicious right axilla during routine mammogram imaging performed on (b)(6) 2024.As per the above information, the date of event was updated.Date of event: april 26, 2024.Along with the initially reported complications, the patient was also diagnosed with bilateral breast ptosis/asymmetry.As an event for the contralateral side was indicated, another file was generated to document the event. This report is for the right breast prosthesis.During the revision surgery, bilateral minimal seromas were discovered.Reason for device explant and/or reoperation: anisomastia. On december 10, 2024, mentor completed a reevaluation of the device per the newly reported complications.Asymmetry may be attributed to one or more of the following: contracture of the fibrous capsule, seroma or hematoma, development of postoperative breast dysplasia, unilateral discrepancy in muscle development, deflation of the implant, incorrect choice of implant shape or size, and surgical technique.Asymmetry is a known complication associated with these devices and is referenced in our current product insert data sheet. Some breast ptosis is a normal component of the mature breast.Ptosis becomes undesirable when the breast parenchyma predominates below the areola, droops considerably below the inframammary fold and the nipple points downward.Augmentation alone, without consideration of the ptosis can produce a less than desirable cosmetic result known as a "rock in sock" deformity or an increased ptotic appearance.Ptosis is a known complication associated with these devices and is referenced in our current product insert data sheet.A seroma is a build-up of fluid around the implant.Symptoms from a seroma may include swelling, pain, and bruising.A seroma may occur soon after surgery, or any time later on, which would be referred to as a late seroma. While the body may absorb seromas, some may require surgery for drainage. The american society of plastic surgeons recommends and encourages member surgeons to always submit breast implants, capsule, and effusion to pathology for examination.Manufacturer¿s reference number: (b)(4).
