Event or Problem Description
It was reported that the video processor was not turning on.The issue occurred during maintenance.There were no reports of patient harm.

This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.Additional information added to fields d8, h2, h3, h4, h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 5 years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's complaint was not confirmed.However, based on the results of the investigation, the malfunction front panel not working and the malfunction no image, it is presumed that occurred due to main board failure.Olympus will continue to monitor field performance for this device.
