On (b)(6) 2024, the patient underwent endovascular procedure using a gore® excluder® aaa endoprosthesis featuring c3® delivery system to treat an abdominal aortic aneurysm. No issues with the device placement or an endoleak were detected at the time of implantation. The patient had a follow up computed tomography angiography (cta) in (b)(6) 2024, and there were no issues noted with the device or its placement.Reportedly, on (b)(6) 2024, the patient was admitted to the hospital acutely with abdominal symptoms. The ct was performed on and the stent was noted to have migrated distally (unknown) and appears to have folded inwards. The graft on acute admission scan was completely collapsed.Reportedly, the diameter of the aorta where the device was positioned remeasured without straightening program using mpr and was 22.5mm right at rentals, 21 mm mid neck and 18 mm just distal. It was reported that the device migration was perhaps due to the undersized graft but the physician did oversize by 15 % lack of supra renal fixation. It was reported that acutely the infrarenal aorta was occluded, and whole graft occluded and collapsed was pulled out of neck basically. The palliative care was done to restore a blood flow. The patient has died despite of efforts to save him and restore blood flow through the aorta.According to the physician, the acute aortic occlusion was cause of death and lower body ischemia.

H.6.Code c19: a review of the manufacturing records for the device verified that the lot met all pre-release specifications. The device remains implanted and is not available for analysis.B5: imaging evaluation was added.According to the gore® excluder® aaa endoprosthesis instructions for use (ifu): read all instructions carefully.Failure to properly follow the instructions, warnings, and precautions may lead to serious surgical consequences or injury to the patient.According to the information reported, the diameter of the aorta where the device was positioned remeasured without straightening program using mpr and was 22.5mm right at rentals, 21 mm mid neck and 18 mm just distal. It was reported that the device migration was perhaps due to the undersized graft.Per gore ifu sizing guide for the rlt231418 - intended aortic vessel diameter (mm)- 19-21.The gore® excluder® aaa endoprosthesis is intended to exclude the aneurysm from the blood circulation in patients diagnosed with infrarenal abdominal aortic aneurysm (aaa) disease and who have appropriate anatomy.Additional considerations for patient selection include but are not limited to patient¿s anatomical suitability for endovascular repair.Ilio-femoral access vessel size and morphology (minimal thrombus, calcium and / or tortuosity) should be compatible with vascular access techniques.Adverse events that may occur and / or require intervention or additional intraoperative procedure time include, but are not limited to: component migration, incomplete component deployment, occlusion of device or native vessel and death.
