Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: it was reported that the augment on demand was prepped via surgical protocol.Kickstand implant would not engage threads of implant.Second implant was put in successfully. The product was returned to j&j medtech orthopaedics for evaluation.Visual inspection revealed the threads stripped of the kickstand augment 6.Additionally, the porous body of device had what appear to be some type of debris. The observed condition of the device is consistent with a component failure that was caused by exposure to unintended forces like overtightening the device while assemblance; or by assembling the device in a misaligned position.Properly handling and attention to the approved use of the device diminishes the risk of failure.Debris on the porous body of device had most likely happen while attempting the implantation.Since this condition does no represent any device nonconformance, a potential cause was not established.A dimensional inspection was performed for the offset taper adapter trial and met specifications*.A functional test was unable to be performed due to the post-manufacturing damage and since mating device was not returned for evaluation. With information provided, and since device met specifications, the reported allegation cannot be confirmed. The overall complaint was not confirmed as the observed condition of the kickstand augment 6 would have not contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.H11 additional narrative: corrected: h3.
