Product complaint # (b)(4).Investigation summary: at the beginning of the day wet equipment is evident, this is reported to the instrumentator and the specialist about the novelty. The specialist decides to work with the team thus, intra-surgically two new features are presented 1- the femoral impactor was disarmed into two pieces which were impacted and with this it was possible to join again, 2- the pin pump when used to remove the pins does not have good grip and together with the specialist should have used a rochester to replace this instrument and so continue with the surgery, 3- twisted femoral fixation pins were evidenced. There were no discomfort on the part of the specialist, the times of the surgery were not affected, nor was the patient affected and the surgery successfully completed, it is left report in the case report and by this means. The product was not returned to j&j medtech orthopaedics; however, photos were provided for review. The visual inspection revealed different instruments which it is not possible to identify the reported device the sp*2 femoral impactor. Therefore, the investigation could not draw any conclusions about the reported event due to the insufficient evidence provided. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was not confirmed as the observed condition of the sp*2 femoral impactor would have not contributed to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a manufacturing record evaluation (nc search) was performed for the finished device product code: 966180, lot: 156927 and no non-conformances / manufacturing irregularities were identified.
