The event was reported via a key monitoring; during an endovascular embolization procedure in the 63-year-old female patient, the 1.50mm x 2.00cm galaxy g3 mini (glm915020 / 30895986) was impeded in the concomitant microcatheter (competitor brand) and could not be further advanced. The physician tried to retract the coil and it was found that the coil had unraveled / stretched. The physician removed the coil and the microcatheter from the patient and replaced the coil to complete the procedure using the same original microcatheter. There was no report of any negative patient impact.A photo of the complaint device and a procedure image were included in the complaint. The photo of the device will be reviewed by the product analysis team and the procedure image will undergo independent physician review. On 12-aug-2024, additional information was received. The information indicated that target aneurysm is on the internal carotid artery (ica) bed segment. The aneurysm is approximately 3mm, regular shaped aneurysm.Per the information, a continuous flush was maintained through the microcatheter. The complaint coil had not been withdrawn and repositioned. There was no resistance between the guidewire and the microcatheter when the target site was being accessed.Prior to the complaint coil, three coils went through the same microcatheter without resistance. The three centering coils were delivered smoothly. The reported issue occurred during the advancement of the complaint coil into the target aneurysm. The microcatheter was not repositioned over the coil. There was no additional damage to the complaint coil aside from the reported stretched / unraveled condition when it was removed. There was no blood flow restriction / reduction as a result of the reported issue. The complaint coil was not replaced; the additional information indicated no replacement, ¿end of embolization step.¿ there was no delay in the procedure as a result of the reported issue. There was no patient adverse event, no negative patient impact. The patient was discharged.
