Event or Problem Description
It was reported, the video system exhibited error in lamp a.There were no reports of patient harm.While speaking with the olympus technical assistance center (tac), the customer was advised to start by changing the lamp.The customer was then transferred to customer service to place an order for a lamp.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Additional information added to fields: d8, h4 and h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 7 years since the subject device was manufactured.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the reported issue could not be determined.Olympus will continue to monitor field performance for this device.
