Event or Problem Description
It was reported that during an unspecified procedure, the subject device had its light cord, handpiece was not working and when plugged into the box, it showed red.There were no reports of patient harm.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the reported issue could not be determined.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor the field performance of this device.
