Event or Problem Description
The complaint/event occurred involved a primary plum set, clave secondary port, 2 clave y-sites, 0.2 micron filter, secure lock, 112 inch. The product was reportedly leaking oxaliplatin during the last few minutes of infusion. The issue/event occurred sometime between 22-jul-2024 and 25-jul-2024.Oxaliplatin was at the y-site into a d5w primary line and the spiros that attaches to the patient began to leak at the site where the primary tubing and the spiros connect. There was no patient harm; however, there was a delay in therapy because they needed to stop the infusion due to leakage.

Additional Manufacturer Narrative
Investigation summary: received one (1) used list #142540489 primary plum set.As received the outlet filter vent of the filter was observed to be wetted out.No additional damage or anomalies were observed. The secondary port clave was activated twice using an icu provided syringe. No stick downs were observed. The set was then leak tested per specifications. There was leakage observed from the outlet filter. The complaint of leakage can be confirmed on the inline filter however not at the secondary port clave. The probable cause of the leak is due to a temporary or complete loss of hydrophobic properties of the filter material due to an infusate interaction during use.The lot history was reviewed, no nonconformities were identified that may have contributed to the reported complaint.
