Event or Problem Description
It was reported that the camera head had cord disconnection.There were no reports of patient harm.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation and due to additional information received.Additional information was provided by the customer: a2, a3, a4, a5 and a6, b5 (per gfe customer confirmed the event occurred during reprocessing), b6,b7, d8, d10 (n/a due to reprocessing)e1, e2, e3, g2, h2, h3, h4, h6 (type of investigation, investigation findings and investigation conclusions) and h11.The device was returned to olympus for inspection and the reported failure was confirmed.In addition, the following reportable malfunctions were identified during the device evaluation: cable flooding and connector contact corrosion.Based on the results of the investigation information suggests probable causes such as damage or deterioration of parts due to wear and tear, without any design or manufacturing issue and/or electrical problems such as signal loss or open circuit.Should additional relevant information become available, a supplemental report will be submitted.Olympus will continue to monitor field performance for this device.
