This report is being sent as follow-up no.1 to provide the completed investigation results. The actual device was not returned; therefore, an evaluation of the actual device was unable to be conducted. The complaint cannot be confirmed for guidewire separation because a sample was not returned for assessment.Without a sample, the exact root cause cannot be determined. The likely root cause is the guidewire was attempted to be withdrawn in the face of resistance due to calcification in the iliac bifurcation, as stated in the complaint description. It is possible the guidewire tip stuck in the calcification and separated during withdraw.Additionally, if the guidewire was withdrawn through the access needle, the guidewire could catch on the needle tip and separate. The ik precision instructions for use (ifu) was reviewed, and it states: "caution: advance the mini guidewire slowly. If resistance is met, do not advance or withdraw the mini guidewire until the cause of the resistance is determined." review of device history record (dhr) cannot be completed due to the unknown lot number.Currently no action is recommended since this risk evaluation is within the predetermined limits in the hazard based risk table (hbrt).
