H3, h6: given the nature of the alleged incident, the device could not be returned for evaluation; therefore, a device analysis could not be performed. The clinical/medical investigation concluded that, based on the information provided, the reported pin tract infections were due to prolonged wear of the external fixator. The patient¿s comorbidities are the likely contributing factors to the delayed healing. It cannot be concluded there was a mal performance of the implant or implant failure of the smith and nephew product.Further impact to the patient beyond the reported could not be assessed due to the patient¿s death unrelated to the fracture injuries & the smith and nephew device.Device batch number was not provided, thus, an evaluation of the manufacturing records could not be performed.A review of complaint history of the previous 34 months revealed a similar event for the listed device, this failure mode will be monitored for future complaints for any necessary corrective actions.A review of the instructions for use documents for external fixation systems reveals in the adverse events that infection including persistent drainage of the pin tracts, or after wire removal; chronic pin/wire site osteomyelitis can occur.A review of the risk management file revealed this failure mode was previously identified. The anticipated risk level is still adequate.A historical review concluded that there are no prior actions related to this product and event.A sterilization records review could not be performed as the batch number was not available. At this time, we have no evidence to conclude that the product failed to meet any specifications at the time of manufacture.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
