Event or Problem Description
It was observed during the device inspection, that the nozzle and the insufflation channel of the colono videoscope are clogged by a dark rubbery material.There were no reports of patient involvement.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the investigation results, the foreign material could not be identified.The foreign material may have been unremoved because the device was not reprocessed properly by the following leaks at the bending section cover and scope cover.However, the root cause for the remaining foreign material could not be established.The events can be detected and prevented in accordance with the following ifu.Ifu states the detection method in gif/cf/pcf-190 series operation manual chapter 3 preparation and inspection.Ifu states the preventive measure in gif/cf/pcf-190 series reprocessing manual chapter 5 reprocessing the endoscope.Olympus will continue to monitor field performance for this device.
