Received one plp1020 in opened original packaging.Lot number was verified.Performed a visual inspection, externally there were no obvious signs of abnormalities or defects.Internally, the coagulation button was concaved making continuous contact with the circuit board.Performed a functional inspection using the esu system 7550 (c8406), the coagulation continuously self activates. The root cause cannot be determined, however, based upon evaluation with photos, the likely cause of this event was excessive force to the button.A two-year lot history review shows a total of 1 device for this lot number and failure mode.A device history review (dhr) was requested from the manufacturer, and no indication of abnormalities was communicated.A two-year review of complaint history revealed there has been a total of 41 reports, regarding 60 devices, for this device family and failure mode.During this same time frame 4,487,235 devices have been manufactured and shipped worldwide.Should all the complaint devices have been found confirmed for this reported failure, the rate of failure would be 0.00001.Per the instructions for use, the user is advised: inspect instruments and cables for damage prior to each use, especially the insulation of laparoscopic/endoscopic instruments. This may be done visually under magnification or with a high voltage insulation testing device.Insulation failures may result in burns or other injuries to the patient or operator. We will continue to monitor for trends through the complaint system to assure patient safety.
