As reported, during an unspecified procedure, a micropuncture transitionless access set's wire unraveled.Access was successfully obtained in the femoral artery with a 21-gauge needle, but resistance was encountered upon advancement of the wire guide. Therefore, the wire and needle were pulled out as one unit; however, the wire unraveled.Reportedly, the access site/femoral artery was calcified and the femoral artery contained atheromatous plaque. The user noted that calcification was a possible cause for the resistance and unraveling of the wire.A cut-down was performed to retrieve the wire tip.The patient is reportedly well.A section of the device did not remain inside the patient¿s body.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.

Summary of event: as reported, during an unspecified procedure, a micropuncture transitionless access set's wire unraveled.Access was successfully obtained in the femoral artery with a 21-gauge needle, but resistance was encountered upon advancement of the wire guide. Therefore, the wire and needle were pulled out as one unit; however, the wire unraveled.Reportedly, the access site/femoral artery was calcified and the femoral artery contained atheromatous plaque. The user noted that calcification was a possible cause for the resistance and unraveling of the wire.A cut-down was performed to retrieve the wire tip.The patient is reportedly well.A section of the device did not remain inside the patient¿s body.According to the initial reporter, the patient did not experience any adverse effects due to this occurrence.Additional information was received 17sep2024.The user reportedly changed the angle of the needle as the wire was pulled back, causing the wire to catch on the needle and subsequently unravel. The tip of the wire did not separate, and there were no adverse effects to the patient. No further information will be provided by the customer.Investigation evaluation: reviews of the complaint history, device history record (dhr), instructions for use (ifu), and quality control procedures were conducted during the investigation.A visual inspection of the complaint device was also conducted. The complaint device was returned to cook for investigation. The wire was stretched and elongated and had started to unravel at approximately 34.1-centimeters from the proximal end.A document-based investigation evaluation was performed.A review of the device history record found no relevant non-conformance's on the final or sub-assembly lots.A review of complaint history found no additional relevant complaints for this lot number. The product ifu states ¿do not attempt to insert or withdraw the wire guide and/or introducer if resistance is felt.¿ and cautions ¿caution: do not withdraw the wire guide through the introducer needle; breakage may result. If the wire guide tip must be withdrawn while the needle is inserted, remove both the needle and the wire as a unit.¿ a review of the device master record (dmr) concluded that sufficient inspection activities are in place to identify this failure mode prior to distribution. The information provided upon review of the dmr, dhr, ifu, and investigation of the returned device suggests that there is evidence the device was manufactured to specification. There is no evidence of non-conforming devices in-house or in the field.Based on the information provided and the results of the investigation, cook has concluded that the patient¿s anatomy and failure to follow instructions contributed to this event. The access site was calcified and the vessel contained atherosclerotic plaque, resistance was encountered upon advancement of the wire, and the angle of the needle was changed as the wire was pulled back, causing the wire to catch on the needle and subsequently unravel, indicating that the wire was manipulated through the needle tip.The ifu cautions that manipulation of the distal portion of the wire through the needle tip may result in breakage. The risk analysis for this failure mode was reviewed and no additional escalation was required. The appropriate personnel have been notified and cook will continue to monitor for similar events. This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
