Event or Problem Description
Medtronic received information regarding an external drainage and monitoring system.It was reported that the proximal catheter was not connected to the 3-way tap and fell out of the packaging during opening/untangling of the catheter.The procedure was completed with back up products.The patient's status was alive-no injury.
Additional Manufacturer Narrative
