Additional Manufacturer Narrative
Product complaint # (b)(4).Investigation summary: it was reported that on (b)(6) 2024, surgeon attached the picture of a ankle fracture repair done by the surgeon.Case date is unknown but from the x ray, surgeon used fibulink and it looks like the fibulink broke where the suture connects the tibial component to the fibula component.Procedure and patient outcome were unknown. The product was not returned to depuy synthes, however photos were provided for review.See attachment mep ankle.Jpg.The x-ray investigation revealed that the washer located on the screw head is loose. Its reasonable to conclude that this position correspond with the lack of tension that should exist between the tibia and the fibula. This condition may be caused by the suture connection the femur and tibial components breaking.According to the surgical technique, page.12, 500709702 rev b tension adjustment advance the tensioning cap clockwise until it is finger tight and the desired level of correction is achieved, which must be confirmed by fluoroscopy.Turn the tensioning knob counterclockwise to decrease tension of the fibulink implant as needed.Use fluoroscopy, direct inspection, and/or ankle range of motion evaluation to ensure an anatomic reduction note: due to this mechanical advantage, a torque limiter has been built into the tensioning knob to prevent applying too much tension to the system. In most instance this limit should not be reached. As the device was not returned, an as-received condition could not be assessed, and a dimensional inspection and document/specification review were not completed. The overall complaint was confirmed as the observed condition of the fibulink syndesmosis repair would contributed to the complained device issue.¿ based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot
