Event or Problem Description
It was observed during the device inspection, the video system center front panel light-emitting diode (led) turned off after some time.There were no reports of patient involvement.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.It has been over nine (9) years since the subject device was manufactured.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, olympus confirmed the occurrence of the event and found that the cable connection of main board was loosened.However, a definitive root cause cannot be determined.Olympus will continue to monitor field performance for this device.
