H10: (b)(4) follow-up emdr for device evaluation: one catheter and orange stylet were received to our quality team for investigation.Through visual inspection, broken tubing observed; therefore, the reported failure could be verified. An orange stylet was inserted through the valve, this is not an approved access method.Per the instructions for use, "drainage procedure - the drainage can be performed using: a) pleurx vacuum bottle(s) b) lockable drainage line with glass vacuum bottle(s) or with wall suction." the inner valves keep fluid from flowing out, unless accessed using access dilator as stated per the instructions for use.A review of the internal manufacturing device records and raw material history files for the reported lot number 0001568480 was performed and no recorded quality problems or rejections related to this incident were found.Product undergoes inspections during manufacturing, no issues related to the reported incident were identified, all procedural and functional requirements for product release have been met.Tubing properties, including tensile strength and elongation are provided by the vendor and reviewed at incoming, no issues were observed.Based on the available information we are not able to identify a root cause at this time regarding the broken tubing.H11: section a through f: the information provide by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
