H11: manufacturing review: a manufacturing review was not requested as the lot number reported is unknown.Investigation summary: the physical device was not returned for evaluation, medical record was provided for review.Medical record review: medical record alleges that, the patient underwent an implantable port placement procedure using a powerport for chemotherapy delivery as part of treatment of colon cancer.Chest x ray was ordered in the recovery to check position and pneumothorax.Five months and seventeen days later, the patient underwent a chest x ray, which revealed that the left chest port had fractured, with the distal portion of the catheter freely mobile. The fragment was noted to be positioned at the level of the superior vena cava and right atrium.Next day, the catheter fragment was removed through the sheath in the right groin. On the same day, blunt and sharp dissection were utilized to free the subcutaneous chest port from its fibrin sheath. The tube was secured and the port and tubing were removed from the patient. Therefore, the investigation is confirmed for the reported catheter fracture, material separation, migration of catheter and the fibrin sheath formed around the chest port.Based upon the available information, the definitive root cause is unknown.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.G3, h6 (method, result, conclusion).Section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
