Product complaint # (b)(4).Investigation summary: according to the information received, ¿it was reported that the mod endo head trials are old and don't fit on the trial stick. These were found after a case so there was no surgical delay and the patient was not affected¿.The product returned to medtech orthopaedics for evaluation. The medtech orthopaedics team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that s/c & mod cath trl 52/28 exhibits an overall worn condition consistent with constant and normal usage for over 20 years.Additionally, scratches were observed at the bottom surface; consistent with other tools and hard edges coming in contact with the device.Properly handling and attention to the approved use of the device diminishes the risk of failure. The lifecycle requirements of the device are event related and depend on the use and inspection of the device in clinical practice. As the device can be damaged on the first or 100th use, the device must be properly inspected prior to each surgical use.Refer to the device/country specific ifu for information related to end of life, reprocessing instructions, and inspection procedures.A functional test performed with a mating device laboratory sample (s/c trial handle / product code: 205512000).The assessment revealed that the s/c & mod cath trl 52/28 was able to assemble and disassemble as intended. Therefore the reported complaint condition "don't fit on the trial stick" was not able to be replicated.A dimensional inspection was not performed as it is not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the s/c & mod cath trl 52/28 would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: a device history record (dhr) review or manufacturing records evaluation (mre) was not possible because the date code (ae0703) provided is not a valid finished goods lot#.H11 additional narrative: corrected: h3.
