Product complaint # (b)(4).Investigation summary according to the information received, ¿it was reported that the event did not happen during a surgery. The long insert for mold is worn and has metal burrs on it¿.The product returned to medtech orthopaedics for evaluation. The medtech orthopaedics team conducted a visual inspection of the returned device.Visual analysis found signs of what appears to be hardened bone cement inside the prostalac lng stm mld insr 165.Additionally, the device exhibits an overall worn condition and burr around the distal edges consistent with normal and constant heavy usage for a long period of time.No other defects were observed.Based on the observed condition, the potential cause of the hardened cement can be attributed to improper maintenance by unintentionally not properly cleaning the bone cement from the mold before solidification.A dimensional inspection and functional testing were not performed as it is not applicable to the complaint condition. The overall complaint was confirmed as the observed condition of the prostalac lng stm mld insr 165 would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot a manufacturing records evaluation (mre) was not performed as no lot number was provided and retrieved for this device.
