Additional Manufacturer Narrative
|

H11: additional information was identified in medical records, and the file was reassessed for reportability and determined to be reportable as death.Manufacturing review: a manufacturing review was not requested as the lot number reported as unknown.Investigation summary: although the physical device was not returned for evaluation, medical record was provided for review.Medical record alleges that, the patient underwent an implantable port placement procedure using a powerport via cephalic vein for chemotherapy delivery as part of lung cancer treatment. The catheter was advanced under fluoroscopic guidance until the tip was resting at the atriocaval junction.Three months and twenty six days later, the patient underwent a ct chest, abdomen, and pelvis. It was observed that the catheter tubing was curled within the right main pulmonary artery. It was identified that the port reservoir was disconnected with the catheter embolized into the right lower lobe pulmonary artery and the right main pulmonary artery. The preliminary x ray demonstrated that the port had completely fractured, and the catheter had embolized the right pulmonary artery. Two days later, the patient was planned for venous port removal.Decedent passed away after one and half years. Therefore, the investigation is confirmed for the reported catheter fracture, material separation, material twisted/bent, and migration issues. The relationship between the port and the patient death cannot be established at this time. However, the patient allegedly expired approximately eighteen months after the port was removed.Based upon the available information, the definitive root cause is unknown.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.B1, b3, b5, d1, g3, h1, h6 (patient, device, method, result).Section a through f: the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.
