Qn#(b)(4).Additional information from the customer states that " the iab failed to unfurl".An additional complaint has been created to capture this malfunction.Associated mdr# 3010532612-2024-00911.Returned for investigation was an 8.5fr sheath w/sidearm & dilator assembly from an insertion kit for a 50cc 8fr ultraflex intra-aortic balloon catheter (iabc). The sample was returned in a ups shipping box and was in a sealed bio-hazard bag.Upon return, the 8.5fr teflon sheath extrusion was noted buckled near the sheath tip; the location of the buckling was at approximate 1.4cm-1.6cm from the sheath tip.The sheath hub and tip appeared typical; no other damage was noted to the sheath. The dilator tip and hub appeared typical with no damage noted.Dried blood was noted within the sheath/dilator.The dilator was removed from the sheath without difficulty. The dilator was aspi-rated and flushed successfully; some blood was noted. The 8.5fr teflon sheath with sidearm was aspirated and flushed successfully; some blood was noted.A device history record (dhr) review was conducted for the lot number with no relevant findings. The device passed all manufacturing specifications prior to release. The reported complaint of "sheath folded in" is confirmed.During the investigation, the sheath body was found buckled near the sheath tip.No other damage was noted to the sheath.Based on a re-view of the device history record (dhr), the product met specification upon release; however, the specifications were not met during the complaint investigation due to the buckled sheath. The root cause of the complaint is undetermined. No further action required at this time. This will be monitored for any developing trends.
