Product complaint # (b)(4).Investigation summary: according to the information received, ¿damaged size 15 broach and broach handle during surgery.Trunion attachment of size 15 broach bent, and broach handle not locking¿.The product returned to medtech orthopaedics for evaluation. The medtech orthopaedics team conducted a visual inspection of the returned device.Visual analysis of the returned corail broach handle revealed that the leaf spring subcomponent of the internal locking mechanism was slightly deformed. The overall appearance of the device exhibits signs of normal wear and no defects that could have contributed to the reported event were observed.A functional test performed with a mating device laboratory sample. The assessment revealed that the corail broach handle was not able to fully lock an hold the mating broach; most likely due to the observed damage of the locking mechanism. The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended forces, such as, heavy usage and applying al excessive amount of force while locking the lever causing the leaf spring subcomponent to bend. The overall complaint was confirmed as the observed condition of the corail broach handle would contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed. There is no indication that a design or manufacturing issue has caused the reported complaint condition. As part of j&j medtech orthopaedics quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
