Event or Problem Description
It was reported that the procedure was performed to treat a lesion in the superficial femoral artery (sfa).During the deployment of a 7x100mm absolute pro self-expanding stent (ses) the thumbwheel was noted to jam causing the stent to only be partially deployed. The physician decided to remove the ses and the introducer sheath as a single unit, however 2mm of the stent were noted to separate in the anatomy. The separated stent was embedded to the vessel wall using balloon angioplasty. There were no adverse patient sequela and no clinically significant delay in the procedure. No additional information was provided.

The device was returned for analysis. The reported stent material separation was able to be confirmed. The reported mechanical jam and the reported activation failure were unable to be replicated in a testing environment due to the condition of the returned device.Production record and corrective and preventive actions (capa) reviews were performed and revealed no indication of a product quality issue.Additionally, a review of the complaint handling database identified no other incidents from this lot.As there was no damage noted to the device during the inspection prior to use, the investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely that during advancement interaction with the anatomy and/or inadvertent mishandling resulted in the noted device damages (jacket stretch marks, kinked stent sheath) preventing the shaft lumens from moving freely; thus, resulting in the reported mechanical jam and the reported activation/deployment failure.Manipulation of the compromised device by disassembling the handle resulted in the noted bent i-beam and the noted bent hypotube likely contributing to the reported difficulties.Further manipulation of the compromised device ultimately resulted in the reported stent material separation/noted stent damages (bent, stretched, broken struts, separated distal stent tabs). As reported, the separated stent was embedded to the vessel wall using balloon angioplasty.Based on the reported information and results of the complaint investigation there is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
