Event or Problem Description
The patient was initially treated for an abdominal aortic aneurysm (aaa) on (b)(6) 2016 with implant of an afx bifurcated stent graft and an afx limb stent graft distal extension.Approximately seven (7) years post initial procedure, the patient presented with a type ib endoleak (of the right common iliac). The physician elected to treat the patient by extending the right limb graft; one (1) additional afx limb stent graft was implanted on 07 august 2023.Reportedly, the patient was in stable condition with no additional patient sequelae.(reported under mfr# 2031527-2023-00024).Now approximately one (1) year post-reintervention on (b)(6) 2024, routine follow-up suspected a type iii endoleak. An angiogram was scheduled for (b)(6) 2024, however it has been postponed until (b)(6) 2024 for unknown reason. The patient status was reported as alive and well.

Additional Manufacturer Narrative
The reported adverse event/incident was investigated in alignment with endologix operating procedures and work instructions.Where possible, it is endologix practice to make at least three good faith efforts to retrieve a reported adverse event/incident related device as well as medical records and medical imaging. An evaluation of the manufacturing record was completed.A review of the part number/lot number combination needed for device identification shows the device to have been properly manufactured and released in accordance with the device master record. An evaluation of the device could not be completed. The device was not returned to endologix for evaluation because it remains implanted.A clinical evaluation of the adverse event/incident was completed. An examination of medical records and/or medical imaging received by endologix found the possible type ii endoleak, possible type iiia endoleak, and possible type iiib endoleak complaints are unconfirmed. This is not consistent with the reported adverse event/incident.Device, user, procedure or anatomy relatedness of this complaint could not be determined.Procedure related harms could not be determined. The final patient status was not reported however, medical records indicate she was discharged home in stable condition following a fall sustaining facial and arm fractures in september. The patient has still not followed up with their physician since then.During the investigation, endologix found reasonable evidence to suggest the device was used off-label.The right common iliac artery maximum diameter was 25 mm at index procedure which is off label since it should be between10-23mm.No additional investigation of this reported adverse event/incident is planned. However, should additional information relevant to the investigation outcome become available, a follow-up report will be submitted.Endologix will continue to monitor this and similar adverse events/incidents.Device iteration is afx2.Corrections: b5: describe event or problem - updated.G3: awareness date -updated.H6: investigation finding codes ¿ remove code 3233.H6: investigation conclusion codes ¿ remove code 11.H6: medical device problem codes - remove code 4065.
