H6 health effect - impact code 4648 was removed.4624, 4607 added. An event of the defective valve and being explanted after 5 years of implant was reported. The investigation at abbott found that all three leaflets remained intact with limited mobility.Biological material was noted throughout the valve. The investigation found cusps 1 and 3 were torn.Fibrous thickening and contained coalesced calcifications which immobilized all cusps. The sewing cuff was excised in processing. There was circumferential pannus formation with narrowing of the inflow diameter to approximately 15 mm.There was no inflammation. The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met specifications. The cause of the torn cusps could not be conclusively determined; however, the degenerative changes noted to the tissue could have contributed to the tear formation. In addition, the fibrous pannus ingrowth noted had the potential to induce increased stress on adjacent cusps and create an unbalanced stress relief distribution between all cusps during coaptation, leading to cusp tear and reduced durability. There is no indication of a product quality issue with regards to manufacture, design, or labeling.
