Additional Manufacturer Narrative
On dec17, 2024, the mentor failure analysis lab received two mentor memorygel breast implants 300cc, catalog number 3543007, lot number 5585293 devices for evaluation.Since it is unclear which is the complaint device, both devices were evaluated.A healthcare professional confirmed unilateral rupture on the right side.A manufacturing record evaluation (mre) was performed, and no anomalies were found related to this complaint. In addition, the mre verifies that the device was manufactured in accordance with documented specification and procedures.Device a evaluation summary: the product was returned to mentor for evaluation.Mentor conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the gel siltex mod-rnd 300cc breast implant had parallel lines of shell wear on both views. In addition, the breast implant was found to be ruptured. The evaluation determined that the cause of the rupture was consistent with normal wear.Shell wear suggests in-vivo folding or creasing of the device. This may be the result of the following factors: continuous and sustained stresses to the device such as too small breast pocket and folding or wrinkling of the shell in the breast pocket. In some cases, the breast implants may also wear out over time.As part of mentor¿s quality process, all devices are manufactured, inspected, and released to approved specifications.Device b evaluation summary: the product was returned to mentor for evaluation.Mentor conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the gel siltex mod-rnd 300cc breast implant was found to have two (2) tears on the posterior view, the tear (a) measuring approximately 0.5 cm and the tear (b) measuring approximately 0.2 cm.Additionally, an area of silex cracking was observed on the edges of both tears. The evaluation determined that the possible cause of both ruptures was consistent with normal wear.Siltex cracking is defined as a material separation occurring in the siltex layer and can eventually progress through the shell of siltex devices.Siltex cracking is ultimately a result of high stresses. As part of mentor¿s quality process, all devices are manufactured, inspected, and released to approved specifications.D4: udi: as the serial number for the device involved in the event was not provided, the full udi is currently not available.Manufacturer¿s reference number: (b)(4).
