Product complaint # (b)(4).Section a1.Patient identifier: (b)(6).The device was discarded; therefore, no further investigation can be performed.A device history review (dhr) associated with this lot presented no issues during the manufacturing or inspection process that can be related to the reported complaint.Cerebral hemorrhage is a known potential complication associated with the use of the embotrap ii device and is listed in the instructions for use (ifu) as such. There was no alleged quality issues related to the used device, as the device performed as intended. The pi assessed the events of ¿bleeding in the left basement area and bleeding on the left pillow leaf¿ as unrelated to the used device but probably related to the procedure. However, since the embotrap ii device was used during the procedure, the correlating relationship between the events to the used device as a contributing factor cannot be ruled out completely.Additionally, the severity is unknown.Based on this information, the events of ¿bleeding in the left basement area and bleeding on the left pillow leaf¿ does meet us fda reporting criteria under 21 cfr 803 with a classification of ¿serious injury.¿ the file will be re-reviewed if additional information is received at a later date.Per the additional information received on 09-oct-2024 and 15-oct-2024, the treating physician considered that the hemorrhagic events were caused by ¿intraoperative excessive perfusion, not related to the specific operation of surgery.¿ the meaning of ¿intraoperative excessive perfusion¿ is unclear, and the procedure details were not yet made available.Nevertheless, the events were assessed as probably related to the procedure and the correlating relationship between the events to the used embotrap ii device as a contributing factor cannot be ruled out completely.Since a cerebral hemorrhage is considered a serious injury, no changes to the previous determination were required.Additionally, it was disclosed that the patient expired on (b)(6) 2024 due to respiratory failure. There is no medical evidence to suggest there is a correlation between the hemorrhage and the event of respiratory failure. The outcomes of the hemorrhagic events were reported as ¿recovering/resolving.¿ therefore, the events are being reported with the classification of ¿serious injury.¿ the file will be re-reviewed if additional information is received at a later date.As part of the post market surveillance program, information from this complaint is trended to identify statistical signals for consideration of further correction action.Since there was no evidence to suggest the event was related to a manufacturing or design issue, no corrective actions will be taken at this time.Missing information from this report is identified as blank; this information was not provided in the reported event or available at the time of report submission. This report is being submitted pursuant to the provisions of 21 cfr, part 803 (and/or part 4, as applicable). This report may be based on information which has not been investigated or verified prior to the required reporting date. This report does not reflect a conclusion by cerenovus, or its employees that the report constitutes an admission that the product, cerenovus, or its employees caused or contributed to the potential event described in this report. If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.
