Additional Manufacturer Narrative
An analysis of the product could not be performed since a physical sample was not received for evaluation.A manufacturing record evaluation was performed for the finished device number lot 31360044m and no internal action related to the complaint was found during the review. If the product is received at a later date, the investigation will be updated as applicable. As part of our company quality system process, all devices are manufactured, inspected, and distributed to approved specifications. This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date. This report does not reflect a conclusion by biosense webster, inc., or its employees that the report constitutes an admission that the product, biosense webster, inc., or its employees caused or contributed to the potential event described in this report. If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.Manufacturer's reference number: (b)(4).

Event or Problem Description
It was reported that a patient underwent an atrial fibrillation (afib) procedure with a thermocool smarttouch and an irrigation issue occurred during use on the patient. The thermocool smarttouch catheter irrigation connection was obstructed.During the procedure, abruptly after doing ablation of right veins, when the physician tried to ablate, the catheter temperature raised to 50ºc and no ablation could be performed.Changed connector and the same occurred. The physician took the catheter outside of the patient to see if the irrigation holes were obstructed. They saw that the catheter was not irrigating; however, no errors appeared. They realized that the irrigation connection of the catheter was obstructed and no flow could pass.Changed the catheter and the error was solved. No patient consequence.Additional information was received. The temperature cut-off value was reached; however, when it arrived at the cut-off, it stopped the ablation. The generator was set to power control mode, temperature threshold: warning was 43ºc, cut off: 50ºc and impedance cut-off: 250 ohms. The occlusion was not due to foreign material or physical damage.
