Additional Manufacturer Narrative
H10.Internal reference number: (b)(4).H3, h6: the associated device was returned and evaluated.A review of the production order did not reveal a manufacturing abnormality that could have caused or contributed to the reported incident. The visual inspection did not reveal any defects. However, a review made by the quality engineering team revealed that during the investigation, it was found that 2 case projects had the same patient's last name associated with them and were mixed.Further investigation revealed that the parts went through the clean process at the same time and swapped when paired back together with the representation orders.A review of complaint history for the previous 12 months did not reveal similar events for the listed device.A historical review concluded that there are no prior actions related to this product and event.According to inspection drawings, the patient name/identifier and lot number on part should be verified to match the print. At this time, we do have evidence to conclude that the product failed to meet specifications at the time of manufacture. The root cause of this event was determined to be a swap between the devices after the cleaning process.Based on this investigation, the need for corrective action is not indicated.Should additional information be received, the complaint will be reopened. No further investigation is warranted for this complaint; however, we will continue to monitor for future complaints and investigate as necessary. We consider this investigation closed.
