Event or Problem Description

|
Additional information received from the physician office clarified that the patient did not experienced inappropriate high voltage therapy during a procedure, rather the inappropriate therapy was on two separate dates and due to atrial fibrillation (af) with rapid ventricular response (rvr.) initially, the af was treated with medication, and then ablation was performed.Subsequent to the ablation procedure, high voltage therapies were temporarily programmed off for three months, which resulted in an audible alert. The programming will be re-evaluated at the end of the three months and the device remains in use.No further patient complications have been reported as a result of this event.
