Event or Problem Description
It was observed that during the device evaluation, the video system center exhibited no image when shaking the connector due to loose lock of video connector socket.There were no reports of patient involvement.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the following led to the malfunction: failure of the video connector socket.Olympus will continue to monitor field performance for this device.
