Event or Problem Description
An attempt was made to use one onyx frontier coronary drug eluting stent to treat a moderately calcified, mildly tortuous lesion with 70 to 80% stenosis in the mid left anterior descending artery (lad). The device was inspected with no issues noted.Negative prep was not performed. The lesion was pre-dilated.The device did pass through a previously deployed stent.Resistance was encountered when advancing the device.Excessive force was not used.It was reported that the stent would not cross into the area needed for deployment.Resistance was not noted during the withdrawal of the device. The stent was removed and upon inspection it was noticed that the stent was not fully seated over the balloon evenly between the markers, as usual. The stent appeared to be off by a millimeter or two.A new same size onyx frontier stent was able to cross and be deployed. The patient was transferred to the icu and expired two days later.
